OBJECTIVE/OVERVIEW:
CUHCC's mission is to seek health equity in our community by
advancing the well-being of diverse people. The clinic is known for
providing high quality care to diverse patients and is moving
forward in improving timely access to care for patients. CUHCC is
building a care model that seeks to improve care at four different
points: before the visit, during the visit, after the visit and
between visits. This position will focus on providing care after
the medical or psychiatry appointment has been completed. This
position will work alongside the providers, interpreters, nurses
and other members of the patient's care team. The initial focus of
this position will be to serve those who are at high risk and who
need immediate follow up upon completion of the visit to ensure
that they have a path to the next steps in their care. This
position will also follow all UMN and CUHCC policies, procedures,
protocols, and manuals as it applies to the duties and assures
compliance and other regulatory requirements.
ESSENTIAL FUNCTIONS:
1. Referral Coordination: (95%)
a) Ensure that all external referrals and orders such as DMEs,
Prior Authorizations, Health Home Care re-certifications, are
complete and properly routed to the appropriate external
provider.
b) Assist in scheduling follow up appointments and/or referral
appointments to other facilities per provider request.
c) Assist patients with maps, facility location and direct dial
numbers.
d) Coordinate with colleagues/Health Information Management to
ensure that all required records, imaging, and/or procedural
results are available to the external provider before patient's
appointment
e) Assist the patient and, or other members of the care team to
ensure patients receive assistance for medications, find
transportation, or connect to community resources (food shelves,
groups, shelters) as needed.
f) Assist providers in completing patient forms (i.e. Medical
Opinion forms, SSI Disability forms) when appropriate.
g) Assist patient in returning to the patient service
representatives to apply for public programs if needed.
2. Other duties as assigned. (5%)
Required Skills:
1. Proficiency in working with computerized medical records,
Microsoft Word, and other software
2. Attention to detail and persistence in completing tasks
3. Ability to multi-task, organize tasks and prioritize
4. High-level communications skills to work with patients, staff,
providers, and outside agencies
5. Excellent customer service skills
6. Respects and works with patients and staff from diverse
cultures, languages, and backgrounds.
7. Capacity to work on a team and make positive contributions to
team efforts.

consistent with the job classification.
A criminal background check is required for this position.
A negative TB test and Hepatitis B Vaccination are required for
this position.
Qualifications
Required Qualifications:
1. Graduate of an approved Medical Secretary program; or one year
of medical clerical experience which must include keyboarding
experience and experience interacting with people (i.e.
receptionist, switchboard, etc.)
2. High school diploma or equivalent.
Preferred Qualifications:
1. Fluency in one of CUHCC's predominant patient languages
(Spanish, Somali, Vietnamese, Lao, Hmong)
2. Aptitude and/or interest in working with both medical and
psychiatry patients in a primary care setting as part of a
team.
3. Training in the Pharmacy field
4. Background in health care and/or medical terminology
5. Experience working in a community clinic
