Receive a $1,000 salary bonus for passing the language proficiency exam!The team leader establishes, administers, and directs the program of Assertive Community Treatment (ACT), a self-contained clinical team which assumes responsibility for directly providing needed treatment, rehabilitation, and support services to identified individuals with serious mental illness or co-occurring mental illness; supervises and evaluates the multi-disciplinary team in conjunction with the psychiatric provider to ensure service excellence and courteous, helpful, and respectful services to program clients; and functions as a practicing clinician on the team.MAIN DUITIES & RESPONSIBILITIES:Schedule staff work hours to assure appropriate coverage for day, evening, weekend, and holiday shifts and on-call hoursLead the daily organizational staff meetings and treatment planning meetingsContinuously evaluate the status of individuals being served by the team and do appropriate planning and coordination of treatment activities to ensure immediate attention to their changing needsProvide oversight to staff development, participate in staff recruitment, interviewing, and hiringDevelop and implement staff orientation and trainingProvide timely performance reviewsDirect, coordinate and schedule the admission process and treatment, rehabilitation, and support services of the program in coordination with the psychiatristDevelop and coordinate the initial assessment and initial treatment planDirect and coordinate, for each individual, the comprehensive assessment of psychiatric history, mental status, and diagnosis; physical and dental health; use of drugs or alcohol; education and employment; social development and functioning, activities of daily living and family structureAssign the most appropriate staff to the individual treatment team (ITT)Provide clinical supervision, for each individual, on the development of the comprehensive assessment and the treatment planProvide leadership in team development and team building activitiesSupervise quality-assurance measures by regularly reviewing assessments, treatment plans and progress notes written by staffCommunicate openly, directly, and professionally with all team membersAssess and monitor services rendered to individuals being served by the teamProvide service coordination to ensure quality care, initiate and maintain relationships, in coordination with other staff, with other service providers, with informal community resources and with individuals' familiesPerform on-call duty for crisis intervention and for staff supervision and consultationIs responsible for working as a team, in concert with each person to develop an individualized plan of services and supports (Individual Treatment Plan) Assist with and/or teach independent living skills such as cooking, cleaning, personal hygiene, transportation, budgeting, and assist clients with making and keeping appointmentsMINIMUM EDUCATION & EXPERIENCE:Master's degree in nursing, social work, psychiatric rehabilitation, OR psychologyLicensed as a mental health professional in the State of MinnesotaPREFERRED QUALIFICATIONS Clear understanding of the characteristics and problems of adults with serious mental illnesses and be knowledgeable about the ACT modelSupervisory and program management experienceStrong clinical skills and experience providing treatment to persons with serious mental illnesses or co-occurring mental illnessesBENEFITS OFFERED BY MHR:MHR is always looking for candidates who are bilingual.
