team leader establishes, administers, and directs the program of
Assertive Community Treatment (ACT), a self-contained clinical team
which assumes responsibility for directly providing needed
treatment, rehabilitation, and support services to identified
individuals with serious mental illness or co-occurring mental
illness; supervises and evaluates the multi-disciplinary team in
conjunction with the psychiatric provider to ensure service
excellence and courteous, helpful, and respectful services to
program clients; and functions as a practicing clinician on the
team.MAIN DUITIES & RESPONSIBILITIES:
- Schedule staff work hours to assure appropriate coverage for
day, evening, weekend, and holiday shifts and on-call hours
- Lead the daily organizational staff meetings and treatment
planning meetings
- Continuously evaluate the status of individuals being served by
the team and do appropriate planning and coordination of treatment
activities to ensure immediate attention to their changing
needs
- Provide oversight to staff development, participate in staff
recruitment, interviewing, and hiring
- Develop and implement staff orientation and training
- Provide timely performance reviews
- Direct, coordinate and schedule the admission process and
treatment, rehabilitation, and support services of the program in
coordination with the psychiatrist
- Develop and coordinate the initial assessment and initial
treatment plan
- Direct and coordinate, for each individual, the comprehensive
assessment of psychiatric history, mental status, and diagnosis;
physical and dental health; use of drugs or alcohol; education and
employment; social development and functioning, activities of daily
living and family structure
- Assign the most appropriate staff to the individual treatment
team (ITT)
- Provide clinical supervision, for each individual, on the
development of the comprehensive assessment and the treatment
plan
- Provide leadership in team development and team building
activities
- Supervise quality-assurance measures by regularly reviewing
assessments, treatment plans and progress notes written by
staff
- Communicate openly, directly, and professionally with all team
members
- Assess and monitor services rendered to individuals being
served by the team
- Provide service coordination to ensure quality care, initiate
and maintain relationships, in coordination with other staff, with
other service providers, with informal community resources and with
individuals' families
- Perform on-call duty for crisis intervention and for staff
supervision and consultation
- Is responsible for working as a team, in concert with each
person to develop an individualized plan of services and supports
(Individual Treatment Plan)
- Assist with and/or teach independent living skills such as
cooking, cleaning, personal hygiene, transportation, budgeting, and
assist clients with making and keeping appointmentsMINIMUM
EDUCATION & EXPERIENCE:
- Master's degree in nursing, social work, psychiatric
rehabilitation, OR psychology
- Licensed as a mental health professional in the State of
MinnesotaPREFERRED QUALIFICATIONS
- Clear understanding of the characteristics and problems of
adults with serious mental illnesses and be knowledgeable about the
ACT model
- Supervisory and program management experience
- Strong clinical skills and experience providing treatment to
persons with serious mental illnesses or co-occurring mental
