Job Description:
Directly responsible to: Director of Nursing/DON Designee
Description: Registered Nurses (RN's) are responsible for the
direct care of the residents, under supervision of Director of
Nursing/Designee, according to facility policies and procedures.
Qualifications: * 1) Has a current Registered Nurse license in good
standing, issued from the State of Minnesota; Maintain current
licensure * 2) Demonstrates safe and competent practice to complete
position responsibilities within the RN Scope of Practice, as
defined by the State of Minnesota. * Keeps updated on new
developments in nursing through continuing education. * Maintain
current CPR certification * Preferred experience in geriatrics and
mental health. * Demonstrate understanding that state and Federal
rules and regulations govern the practices within the facility. *
Ongoing pursuit of Continuing Education in clinical subjects,
personal growth and development. * Demonstrate working knowledge of
infection prevention and control principles and practices. * Proven
ability to communicate effectively and positively with staff,
residents, family and others. * Must be capable of maintaining
regular attendance. * Must meet all screening requirements: pass
drug screening, (if applicable), pass criminal background
investigation and reference inquiry. * Must be capable of
performing the essential job functions of this job, with or without
reasonable accommodation. * Position Responsibilities: * Receive
report from off-going nurse/give report to oncoming nurse.
(Including a discussion of each resident, making notations of
significant changes and new residents.) * Count Schedule I-IV
controlled substances with the on-coming and off-going nurses. *
Read PCC shift report/progress notes since the last shift worked to
keep updated on changes. * Check appearance of any ill or injured
resident. Contact appropriate medical provider as needed. Provide
treatment as ordered and document in medical record. * Administer
routine and PRN medications and treatments as ordered. Document on
the administration record/PRN sheet. * Obtain/record/review routine
vital signs & weights. Notify DON of significant changes. * Inform
MDS nurse or DON of updates to maintain residents' care plans as
needed. * Document pertinent information on residents' medical
record according to policies and procedures: a) 7 day observations
charting (every shift); b) Follow up documentation (see flagged
charts); c) change in health status * Check upcoming appointments
in appointment book. Ensure resident is aware of appointment and is
bathed, appropriately dressed, and ready on time. * Assist with
residents' personal care, hygiene, and ADLs as needed. * Ensure
that residents come to meals on time and are assisted as needed. *
Ensure that all residents are well groomed and appropriately
dressed. * Order medications, sign in medications from Pharmacy. *
Write referrals/lab slips. * Take telephone orders from physician
and transcribe orders. * Order lab studies; report any abnormal
values to the DON and physician. Send copies to physicians. *
Complete incident reports when needed. * Lock and unlock door at
specified times. * Answer the phone. When business office is closed
- take messages & deliver messages appropriately. Check nursing
office voicemail every shift and address messages. * Check on
ill/injured employees and follow facility policies and procedures.
* Notify DON and Administrator of any emergency/disaster situations
immediately. * Keep residents' physician and family informed of
changes in condition. * Notify dietary department of dietary orders
and changes. * Ensure that residents' rooms are neat and orderly. *
Maintain safety of the residents and security of the building.
Report/correct actual or potential hazards. * Maintain nursing
office and carts in neat and orderly manner for up-coming shift. *
Attend In-services and staff meetings. * Monitor the work of CNAs,
TMAs. Keep DON informed of any issues. * Respond appropriately to
emergency situations. * Follow Abuse Prevention Policy and
reporting requirements * Knowledgeable and competent of facility
regulations and internal policies. * Assist
visitors/family/friends/physicians/lab/transportation personnel as
needed. * Transcribe physician, NP, psychiatrist orders after
visits have been made. * Admit/discharge residents according to
facility policies and procedures. Notify DON of any unplanned
admit/discharges. * Request/dispense nourishments. * Make rounds;
breathe checks every hour. * Wake residents for toileting programs,
per schedule. * Perform light cleaning/housekeeping tasks as
needed. * Restock supplies and carts * Empty commodes/urinals *
Perform diabetic orders/cares and maintain equipment as needed *
Notify office and/or DON for more copies of forms or logs needed. *
Any other duties as assigned by the DON or Designee Job

necessary medications after med pass? Controlled substance count?
Documentation of 7 day observation, behavioral episodes and
redirection, refusals, etc? Review and update resident care plans
as needed? Audit medication/treatment carts? Light housekeeping,
sanitation, disinfection as needed? Train new employees as assigned
by staffing manager, DON, DON designee? Attend Nursing Meetings?
Follows facility policies and procedures and CMS regulations? Shift
specific tasks as assigned? Other duties as assigned by or DON/DON
