As a PT in Home Health at AccentCare, you will play a crucial role in helping patients regain movement and manage pain from the comfort of their homes. Our ideal candidate will be a detail-oriented, passionate professional who adheres to the highest standards of physical therapy and patient care. This role involves assessing patient needs, crafting personalized care plans, and implementing therapeutic solutions to improve patients' daily lives. Evaluate and assess patient needs and condition. Develop and implement tailored care plans based on each patient's goals and evaluated needs. Monitor patient progress and adjust treatment plans accordingly. Educate patients and families about health preservation and disease prevention. Maintain patient records and document therapy progress. Communicate regularly with healthcare team members. Strong interpersonal and communication skills. Expert knowledge of therapeutic exercises and techniques. Ability to operate standard physical therapy equipment. Strong analytical and problem-solving skills. Medical Insurance Dental Insurance Vision Insurance 401k Current state PT licensure. Valid driver’s license and reliable transportation. CPR certification.
