Position Overview We are seeking a Hospice Case Manager to join our
dedicated team. This role involves planning, organizing, and
delivering hospice care with a strong emphasis on community health
education. As a hospice RN, you will coordinate care for patients
and their families, ensuring comfort, dignity, and quality of life.
Key Responsibilities Patient Care Conduct comprehensive initial and
ongoing assessments of patients and families to determine hospice
care needs. Develop, implement, and adjust individualized care
plans based on patient conditions and evolving needs. Administer
medications, treatments, and palliative care interventions as
prescribed. Educate patients and families on symptom management,
medication use, and self-care techniques. Provide emotional and
psychological support to patients and their loved ones. Assist with
activities of daily living to enhance patient comfort and
independence. Serve as a Case Manager, coordinating care for
assigned patients. Communication & Collaboration Maintain accurate,
detailed clinical records and document changes in patient
conditions. Collaborate with physicians, caregivers, and
interdisciplinary team members to ensure seamless care. Participate
in family and team meetings, providing updates and guidance on care
plans. Advocate for patient needs and facilitate access to
necessary resources and support services. Additional
Responsibilities Participate in on-call rotations as needed. Ensure
necessary medical equipment and supplies are available for patient
care. Supervise and support hospice aides as needed. Engage in
professional development and continuing education. Contribute to
quality improvement initiatives within the organization.
Qualifications Graduate of an accredited School of Nursing Active
RN license in the state of Minnesota CPR certification required
Minimum of 1 year of acute care experience (institutional setting)
and 1 year of home care or hospice experience preferred Strong
clinical assessment, critical thinking, and problem-solving skills
Compassionate and patient-centered approach to care Ability to
lift, position, and transfer patients as needed Must have a valid
driver's license and insured vehicle for travel to patient
