In this role, you will oversee the care of our clients, conduct nursing assessments, and collaborate with caregivers to ensure clients receive the best possible care in the comfort of their homes. Reports To: RN Supervisor Key Responsibilities: Travel to clients’ homes to perform nursing assessments and observations, documenting findings accurately Regularly evaluate clients to ensure their needs align with their care plans Develop, monitor, and update individualized care plans in collaboration with the care team Provide guidance and support to caregivers, ensuring that they deliver quality care in accordance with care plans Address client concerns, making sure patient rights are respected and upheld Offer ongoing coaching to caregivers, reinforcing proper safety procedures and best practices Collaborate with management to revise operational procedures and ensure quality client care is delivered consistently Act as a mentor to staff and foster a culture of professionalism and excellence in caregiving Qualifications: Experience: At least 1 year of home health care experience preferred Skills: Comfortable with technology and software systems (experience with scheduling software, Wellsky, Microsoft Office 365, etc. is a plus) Team-Oriented: Ability to work effectively within a team to deliver the best outcomes for clients Strong ability to maintain confidentiality and communicate effectively with staff, clients, and families Ability to handle challenging situations with professionalism and empathy, including occasional verbal or physical mistreatment from clients Experience supervising and mentoring caregivers, ensuring high standards of care Excellent verbal and written communication skills, with the ability to adapt to different situations and client needs Strong multi-tasking skills and ability to work independently in a fast-paced environment Education & Certification Requirements: Bachelor of Science in Nursing (BSN) Current RN Licensure Current TB Test Must maintain CEU requirements as specified by the Board of Nursing Valid Driver’s License and reliable transportation Why Senior Helpers?
