FUNCTIONS/RESPONSIBILITIES: Conduct initial and ongoing
comprehensive assessments of patients and families Perform physical
evaluations and document medical history Develop, initiate, and
revise individualized plans of care Use health assessment data to
determine nursing diagnoses Include patients and families in the
care planning process Administer medications as prescribed Provide
health education and counseling to patients and families Assist
with daily living activities and promote comfort care Serve as RN
Case Manager for assigned caseload when designated by Clinical
Supervisor Coordinate care and collaborate with team members to
ensure continuity Clinical Documentation & Communication Accurately
complete, maintain, and submit timely clinical notes reflecting the
patient’s condition and care provided. Document changes in pain and
symptom management, including outcomes and interventions.
Communicate proactively with physicians regarding patient needs and
condition changes; obtain and implement physician orders as
required. Coordinate care by collaborating with community health
professionals and resources. Patient Care & Education Provide
medications, dietary guidance, and other instructions as prescribed
by the physician. Identify and utilize opportunities for comfort
counseling with patients, families, and caregivers. Deliver care in
alignment with the interdisciplinary team to ensure holistic
support. Safety & Support Maintain a safe, clean, and supportive
environment for patients at all times. Assist patients, families,
and team members in delivering compassionate, comfort-focused care.
Support the emotional and psychosocial needs of patients and
families in collaboration with the hospice team. Team Collaboration
Actively participate in interdisciplinary group (IDG) meetings to
ensure coordinated, patient-centered care. Requirements:
Qualifications: We’re looking for a compassionate and skilled RN
Case Manager who meets the following criteria: Graduate of an
accredited school of nursing Minimum 1 year of experience as a
hospice case manager (preferred) Current RN licensure in the state
of practice Strong observation , verbal and written communication ,
and problem-solving skills Proficient in basic math and core
nursing competencies Comfortable with prolonged walking or standing
Able to lift, position, and transfer patients , as well as supplies
and equipment Physically capable of reaching, stooping, bending,
kneeling, and crouching Possess the visual acuity and hearing
required to perform essential nursing tasks Must be a licensed
driver with a reliable, insured vehicle that meets state and
