• Collaborate with the interdisciplinary team to develop and implement individualized care plans.
• Provide direct care to residents, including administering medications, performing treatments, and documenting resident conditions.
• Lead and direct nursing assistant assignments to ensure efficient and effective care delivery.
• Communicate effectively with residents, families, and healthcare providers to ensure seamless care transitions.
• Participate in quality improvement initiatives to enhance resident outcomes and satisfaction.Requirements
  • Graduate of an accredited Licensed Practical Nurse program.
