Overview:Our clinical model supports your professional judgment

tools and logistical support to allow you to focus on clinical
care. As a Triage & Transition team member there are opportunities
to provide care in a variety of settings.This person in this
position will focus their time as a collaborative team member with
the EmPATH team at Fairview Southdale. The individual in this
position will complete crisis and diagnostic assessments,
therapeutic interventions, group therapy, provide family
interventions, and work to create and direct patient care and
disposition plans. Building and strengthening relationships will be
a foundational part of this work, as this individual will work
closely with psychiatry, medical providers, and the
interdisciplinary team.Our team has been working to positively
impacting the community since 2002. We help youth and adults
receive timely and compassionate care during periods of crisis. We

transforming the ways in which crisis assessments and behavioral
health services are available through technological innovation and
community partnerships.This position is an on-site role at MHealth
Fairview Southdale on the EmPATH Unit.Our Triage & Transition Teams
work to ensure our patients get timely mental health and addiction
care in the least restrictive environment, focusing on therapeutic
service delivery through comprehensive assessment, intervention and

The New Yorker!Responsibilities/Job Description:Clinical
Services:Provides coordinated care by initiating client centered
consultation with internal and external providers involved in care
or services. Responsible for obtaining the necessary
releasesProvides crisis call support and determines appropriate
level of care, as needed, for clientsCompletes a comprehensive
behavioral assessment in person or virtually via telehealth to
identify patient needs and to recommend a level of careProvides
psychotherapy for clientsAssesses and completes the DEC (Diagnostic
Evaluation Center) assessment form for all crisis
patientsDetermines appropriate disposition, coordinates care, and
provides discharge summaries for patientsCollaborates with the
patient, medical doctor, and family to create a treatment plan
based on medical necessity and the patients preference for
careProvides primary communication to collaborating partners, e.g.
Hospital emergency departments, schools, primary care clinics,
assisted living, and other treatment providersAdheres to all
policies and proceduresMaintains knowledge of available community
resources and procedures for hospitalizationsRecord Keeping:Obtains
all necessary documents, information, and signatures from patients
in order to complete assessments and coordinate careConducts risk
assessments, de-escalation techniques/suicide prevention, and/or
mental status evaluationPromptly and accurately documents patient
information, the major crisis or treatment issues and expected
outcomes based on medical record-keeping standards.
Documents/charts according to department standards, policy and
regulatory and accrediting bodies requirementsService to Internal
and External Customers:Responsible for responding to queries
pertaining to services that can be provided to best meet the needs
of the patientTroubleshoots technical issues with telehealth
sitesLocates and schedules routine and crisis appointments for
patients within the established timeframeEstablishes and maintains
positive and professional working relationships with internal
staff, external service providers and other state and county
agenciesEngages in appropriate professional development through
continuing education, membership in professional organization and
licensure as eligibleAs requested, participates in nonclinical
activities such as committee work, quality improvement teams,
performance improvement and grant
initiativesQualifications:Required:Masters Degree, from an
accredited school, in Social Work, Psychology, or closely related
disciplineBasic Life Support CertificationValid Minnesota drivers
license, access to a car during designated hours, proof of auto
insurance coverageLicensed in one of the following in the State of
Minnesota:Licensed Psychologist (Doctoral or Master's)Licensed
Independent Clinical Social WorkerLicensed Marriage Family
Therapist (Doctoral or Masters)Licensed Professional Clinical
Counselor (Doctoral or Master's)Good working knowledge of
electronic health records as well as a comfort level and
willingness to utilize computer-based platforms for assessments
along with proficiency with computersAbility to handle multiple
tasks at one time and to quickly switch from one task to
anotherPreferred:Three or more years experience in crisis
counseling/situations or clinical therapyOne to three years
leadership experience in a behavioral related field, managed care
