As part of Fairview's Community Health and Wellness Hub, EICR is committed to delivering innovative, collaborative care. The Fairview Community Health and Wellness Hub opened in 2022 to deliver a variety of services to the communityfrom primary and mental health care to enrichment options for seniors, food access programs, and community gathering spaces. The Hub brings Fairview services together with local organizationsincluding Minnesota Community Care and Sanneh Foundationto make it easier for people to access what they need to be well.Responsibilities/Job Description:Monitors personal cares provided to assure consistency with each individual plan of careAdministers medications and performs treatments per physician orders and facility policiesProvides data relevant to the development of the plan of careRN Assesses plan of care, resident goals and revises, modify plan of care as necessaryProvides reports and documentation related to incidents, change of status and cares providedQualifications:Required:Vocational/Technical Training or Associate Degree - Registered NurseCurrent MN RN licenseAHA BLS for Healthcare Providers or Red Cross CPR for Professional RescuersBLS is required upon hire and/or new hire must show proof that they are currently enrolled in a BLS class to renew or obtain their certification. BLS certification must be obtained within three-weeks of hire date and before nurse can perform independent practice0-1 years of experiencePreferred:0-3 years of experienceTCU LPN/RN experienceBasic computer skillsPoint Click Care knowledgeby Jobble
