Overview:M Health Fairview is looking for a dedicated Registered Nurse (RN) on the Long-Term Acute Care Hospital unit (LTACH) at Bethesda Hospital in St. Paul, MN! This role is eligible for full benefits including: medical, HSA, dental, vision, 403b, PTO, Shift Differentials, health & wellbeing resources, tuition assistance & reimbursement and more!Schedule: .4 FTE (32 hours pre two weeks) 8 hour shifts on the evening schedule (3pm-11:30pm) Working 5 out of 6 weekends (Saturday & Sunday)The Bethesda LTACH is located in the former St. Joseph's facility. This unit is an excellent place to build expertise in handling patients with progressive care issues at varying levels of multi system impairment. The LTACH is an inpatient unit where all our patients have had a complex illness or injury that requires hospital level care for an average of 25-30 days. Many of our patients are more acute than a typical Med/Surg floor.Responsibilities/Job Description:Responsibilities Job DescriptionThe RN serves as the key leader of the interdependent team and is responsible to achieve patient and unit outcomes by coordinating the care for a group of patients. This includes: comprehensive assessment of patient and family needs; identification of outcomes based on the needs' assessment; establishing the plan of care; supervising the progress; and modification of the plan of care as needed.Demonstrates commitment to principles of the patient care model (Professional Partnership Model) and achieving the Bethesda LTACH nursing vision and strategyApplies the nursing process, including assessment, planning, implementation, and evaluation, to coordinate outcome focused patient care. Throughout this process, also understands and incorporates holistic nursing theory based on Watson's Theory of Caring aligning with the practice standards of the American Holistic Nurses' Association whenever holistic therapy is offeredCommunicates efficiently with patient/family, physicians, the interdependent team, and other disciplines regarding the plan of careAssesses and documents learning needs of the patient and family. Develops an individualized education plan for patients and families based on identified needsActively participates in crafting and implementing improvements at the point of care to achieve clinical quality, customer experience, nursing engagement, and efficiency and effectiveness outcomesComplies with regulatory requirements, accreditation standards, board of nursing requirements, patient safety requirements, infection control practices, and other established practice standards, policies, guidelines, and proceduresCoordinates the interdependent team while promoting collaborative teamwork and cooperationActively participates in the development and achievement of department goals (safety, cost, quality, patient experience, employee engagement)Maintains knowledge of, and complies with, all relevant laws, regulations, policies, procedures and standardsQualifications:Required Qualifications:One year RN experienceBasic Life Support Through Health Care ProvidersDegree in NursingRN LicensePreferred qualifications:Telemetry required within 1 year of hire, receive training on unitBSN degree
