As an Admissions Coordinator, you will:
Contact the patient and family within 1 hour of referral, and
coordinating all admissions within the 3-hour admission
Process referrals by obtaining sufficient admission and insurance
information in order to ensure appropriate and timely admission of
Maintain communication with pending patients, their families, and
referral sources and routinely follow the progress of prospective
patients and those referrals not taken under care (NTUC).
Participate in morning Stand-Up meetings.
Participate as a member of the marketing team by: actively seeking
new referral sources; developing and maintaining positive
relationships with referral sources; assist in developing
strategies and plans for Site and Company growth; participating in
marketing meetings, staff meetings, department meetings, staff
meetings, in-services, committees, community-education events and
other activities as needed.
Prepare marketing reports regarding referral statistics. Ensure
high level of integrity of referral data within Company computer
Maintain working knowledge of, and assist in, accessing community
Participate in evening/weekend on-call or overtime as required.
Perform admissions presentations in the field and via phone, as
directed by ED, to assist admission team in meeting 3-hour
Ensure maximum reimbursement through initial payer source,
benefits, and eligibility verification, authorization processes,
and accurate data entry into Company computer system. Completes
required payer source verification forms.
Strong customer service experienced
Hospice or medical office experience highly preferred
LPN/LVN certification or Bachelors degree in business, social work,
psychology or nursing or equivalent business experience
Able to work in an interdisciplinary setting
Ability to maintain business and clinical files in a systematic and

Proficient with the PC including the MS suite of products
