Job Description:
SUMMARYThe Registered Nurse (RN) - Central Float Staff provides
relief staffing resources to our Long Term Care, Transitional Care
and Assisted Living communities. As a RN this position will (a)
assist each resident in achieving and maintaining their highest
practical level of physical, mental, and psychosocial well-being
through providing medication, treatment administration and quality,
personal care; and (b) utilizes the nursing process in planning,
directing and providing direct care to residents in accordance with
professional nursing practice standards, established policy and
procedures, individual plan of resident care and regulatory
requirements.
PRIMARY RESPONSIBILITIES
- Provide direct care to residents under individual plans of
care, administering care and medications per doctor's orders.*
- Act as an advocate to maintain client privacy and dignity
during cares. Administer prescribed medications and treatments per
doctor's orders.*
- Monitor resident vitals, completing resident assessments as
assigned. Communicate any identified changes to physician and
family members.*
- Collaborate with the IDT (interdisciplinary team) to ensure
adequate and appropriate flow of services.*
- Assist throughout the admission process, to ensure smooth
transition of new residents.*
- Serve as a clinical resource to staff regarding nursing care of
residents. Provide in-service education as requested.*
- Maintain and complete all required documentation and charting
on the computer or on forms in an accurate and timely manner
according to policies and procedures. *
- Accurately transcribe Physician's Orders as per facility
protocol and forward necessary information to other departments as
needed.*
- Initial Medication Administration Record (MAR) after medication
is administered per facility policy and procedure, accurately
documenting refusals for medication on back of MAR.*
- Support various other Nursing responsibilities as needed.
Additional Care Center Responsibilities
- Supervise dining room activities, assisting residents with
eating as needed.*
- Oversee and lead nursing staff, including LPNs, NARs, and
TMAs.*
- Communicate expectations and ensure that unit personnel are
carrying out their assignment.*
- Organize and provide work direction to unit staff.*
- Facilitate conflict resolution between residents, staff,
families, community and other disciplines (Hospice, Therapy, M.D.s,
Nurse Practitioners, etc.)*
Additional Homecare Responsibilities
- Complete comprehensive assessment for pre-admissions, new
admissions, hospital transfers and discharge.*
- Supervise, oversee and lead unlicensed staff, ensuring
Minnesota Home Care Rule and Statute requirements are met.*
- Communicate expectations and ensure that unit personnel are
carrying out their assignment.*
- Organize and provide work direction to unit staff.*
- Ensure staffing needs are met in collaboration with the
Staffing Coordinator.*

JOB SPECIFICATIONSEducation, Experience and Credentials
- Current licensure in the State of Minnesota as an RN
required.
- Current CPR certification required.
- One or more years' experience in a long-term care or
transitional care setting preferred, six months required.
- Strong clinical assessment experience preferred.Knowledge,
Skills and Abilities
- Flexible and productive in an environment with regular
interruptions.
- Display high levels of customer service, responding promptly
and thoroughly to the inquiries and needs of individuals both
internal and external to the organization.
- Understand and maintain confidential nature of organization and
client information in accordance to HIPAA regulations and corporate
compliance standards, including appropriate access of data housed
within the Information Technology infrastructure.
- Possess effective problem solving skills.
- Strong computer skills, including Microsoft Word, Excel,
Outlook, Internet, and other applications. Must be skilled in
record keeping.
- Able to display high levels of customer service, responding
promptly and thoroughly to the inquiries and needs of individuals
both internal and external to the organization in accordance with
our PROUD standards.
- Able to understand and follow facility and department safety
and emergency procedures.
PHYSICAL REQUIREMENTS NECESSARY TO PERFORM THIS JOBIncumbent must
be able to:
- Perform medium work, lifting up to 50 pounds, and occasionally
lifting/carrying of objects weighing up to 25 pounds.
- Push/pull people and equipment up to 50 pounds a distance of
several feet.
- Twist, turn, bend, stoop and reach frequently in the
performance of client assistance responsibilities.
- Effectively communicate, including following oral and written
instructions, in order to communicate with people inside and
outside the organization.
- Able to effectively use office equipment to review and create
information.
- Work at a desk for periods of time and perform on a
computer.
- Frequently stand and walk to move around office/building
repeatedly throughout the day.
- Able to work a flexible work schedule, including overtime,
nights and weekends as required including attending events/meetings
as needed.
- Able to obtain reliable transportation.
TOOLS AND EQUIPMENT USED
- Use thermometer, stethoscope, blood pressure cuff, wheelchair,
patient lifts, and related medical equipment.
- Use telephone, computer and standard office equipment.
WORK ENVIRONMENTThe work environment is indoors. The incumbent must
be flexible about switching positions quickly in order to respond
to the needs of residents, in responding to emergencies. May be at
risk of exposure to blood, bodily fluids, and other potentially
