KEY POSITION SPECIFIC ROLES & RESPONSIBILITIESRole models positive communication skills with all staff, residents, and families.Participate in shift report and keep open lines of communication with supervisor, co-workers and on-coming shift.Attend nurses and team meetings as scheduled and/or assigned.Responds to all emergency situations within the facility.Encourages open discussions with residents and resident families.Promotes rehabilitation nursing on the units.Performs nursing assessments on residents when necessary.Reports all unusual circumstances that may involve the safety and well-being of the residentsand staff directly to the DON.Demonstrates leadership ability and management skills through fair decision making, dependability and reasonable expectations.May chart and/or co-sign shift documentation for nursing.Participates in the community continuous quality improvement program.Serves as a resource to nursing personnel, other departments, families, residents, and other health care providers.Assists with chart audits for regulatory compliance.Attends educational meeting and seminars to promote personal and professional growth.Keeps the Director of Nursing informed of relevant issues in areas of expertise.Knowledgeable of emergency/safety procedures and policies and directs/assists staff and residents as the need arises.Understands and adheres to State/HCFA/OSHA regulations.Serve as directed on various quality assurance committees of the facility and provide written and/or verbal reports of the activities as required by the committee guidelines or directions.Supports Nursing Administration in their absence.Participates in nursing on-call rotation per facility policy and will work weekends and holidays according to nursing schedule.Performs other duties as assigned.JOB REQUIREMENTSSuccessful completion of a Registered Nurse or Licensed Practical Nurse (RN or LPN) program at an accredited school, college or university. Registered Nurse with minimum of 2 years of experience in nursing in a long term care orrehabilitation nursing.Demonstrates knowledge of the aging process, rehabilitative and medical-surgical nursing care.Overall understanding of MDS/RAI process, Case Mix, and Medicare/PPS.Knowledge and experience in member assessment, developing care plans, utilizing the nursing process and coordinating interdepartmental efforts in achieving quality outcomes.Strong clinical skills including assessment and treatment interventions for a variety of diseases/illness.Broad based health care operational expertise with understanding of financial analysis, negotiations with both oral and written communication.Must have and maintain an active R.N. license/registration in good standing on the state registry.Current CPR certification.Certified Geriatric Nurse by ANA, preferred.As a provider of quality care and services, we are proud to align with healthcare's best practices to protect the health of our residents and team members.
