    Job Summary

    This position exists to lead in the design of transition and case management services and supports for DHS's federally funded Money Follows the Person (MFP) Rebalancing Demonstration Grant. Minnesota's MFP grant is known as "Moving Home Minnesota" or MHM. The MFP project is a systems transformation grant designed to "rebalance" Minnesota's Medicaid system and home and community-based services (HCBS).

    In alignment with targeted case management services, the individual will serve as MHM's lead in transition coordination, relocation service coordination, and case management services policy, as well as the legislation, strategic planning, technical assistance, and operational work. The individual will ensure that MHM meets the federal state and waiver plan requirements for relocation service coordination and case management, and the MFP grant requirements for the demonstration services of transition coordination and case management. Additionally, the individual will ensure that MFP, MFP Tribal Initiative, and DHS identifies and fulfills evaluation and continuous improvement needs and contributes to the system-wide efforts to promote the rebalancing of Minnesota's long-term supports and services.

    Responsibilities include, but are not limited to:
    • Develop, lead, and direct MHM's policies and HCBS development for transition coordination, relocation service coordination, and case management. Develop demonstration and supplemental transition services and supports that align with the MFP/MHM mission and objectives.
    • Lead the MHM team in the ongoing day-to-day operations of institutional to community transitions, and demonstration functions and initiatives (e.g., eligibility, enrollment, care/service plan development, service use, and outcome measurement, service delivery and operational capacity development) to ensure efficient operations and alignment with strategic goals and ensure that demonstration activities are carried out without interruption.
    • Identify and promote strategies that improve the equitable administration of transition-related supports. Build capacity and improve the delivery of transition coordination, case management, support planning and informed choice.

      Qualifications

      Minimum Qualifications

      Three (3) years specialized professional experience in transition or relocation services case management for a human services or community-based services program.

      Experience must include:
      • Knowledge of Medical Assistance, Money Follows the Person/Moving Home Minnesota, home and community-based services and programs;
      • State and local service delivery systems and funding streams for older adults, adults and children with disabilities, mental health and chemical health treatment, and housing supports;
      • Practical understanding of Minnesota's transition coordination services and supports, and case management systems and processes;
      • Familiarity with interpreting legislative processes, procedures, and protocols that have broad and interrelated impact.
        Preferred Qualifications
        • Variety of experiences working effectively with others from different backgrounds and cultures;
        • Strong human relations and communication skills; communicate clearly and professionally both verbally and in writing; respond openly to requests and concerns;
        • Project Management skills;
        • Facilitator of groups to guide broad, agency-wide collaboration, quality assurance and improvement initiatives.
          Additional Requirements
          • The Minnesota Department of Human Services (DHS) will conduct criminal background checks on all finalists for this position. Finalists must pass the DHS background check.
